i think this is a load of feathers butt to be honest i was prestige 4 just prestiged, had diamond rocket launchers -.- for those who have it will know the struggle with rpg and fhg with the skull challenge with the controlled air craft was a pain in the butt and almost had diamond smg, i got deranked but carried on playing i hit 1st prestige and got deranked again, now i know this is no accident and trying to get this sorted is harder because i cant get through to the form aswell so hope this gets sorted quick

Man, it's really really a bummer seeing that I'm a Zero prestige corporal without the ability to change my scorestreaks

Same here played to level 46 5th prestige had submachine gun in diamonds was at a friends house logged in in split screen with him the game froze we reset the ps3 i log in and my rank was at 1 leader board standing still correct just erased everything else this has ruined the game for me im not getting my hopes up that they will give me everything back it it just pisses me off that i have 6 days playing the game all for nothing

same thing happened to me aswel, it can be resolved but you have to link all multiplayer accounts to your profile, which i'm having trouble with! :/ can anyone tell me how to do this?

The solution in my case was to delink my xbox live account and link it new.

Now i can enter the page.

These guys are a bunch of crap, I have called in multiple times and after being put on hold for over an hour I have been hung up on and treated like crap. My stats were never restored and in the process of doing this, my husbands guns were reset and rolled back. The form will not sync to my ps3 muliplayer account and can not fillout the form. The last time I called they even told me explorer and safari browsers dont work. when i said i was using firefox, they hung up. I am about to stop playing and using activision/treyarch games. no way to run a business.

Im having the exact same problem, first all my stats get reset and know i cant evn get to the reset stat form. Gettin really fustrated.

Same problem i was prestige level 4 level 7 and just got my diamond snipers and i was excited, you guys know the feeling working hard to get it, then i logged into local to play with my little sister then i go online and im prestige 3 level 52 and all my diamond snipers are gone and the kills for the last one gone i was like omfg so i go to the forums and its like you have to link multiplayer account(s) to profile im already linked! im part of elite as well i tryed everything im on xbox 360 and my gamertag is RATED DIRTYWORK so please help us!!!! i cant even go online and play im so dissapointed

unlink your acc and relink it back

I FOUND THE SOLUTION. like other people have said just unlink and relink the account back and it should work. that's what i did and i just sent my form in. hope this helps

I've had the same issue, lost everything and couldnt get into the Stats Reset page, kept saying need to link Multiplayer Accounts to Profile which it cleraly was but still wouldn't work.. I 'Un-Linked' my PS3 and re-linked it again and tried the Stats Reset Page again, this time it worked.. Whoop!
