Based on Activision's Banning Policy, a Third Offense for Boosting, Cheating and Glitching, or a First Offense for Hacking is a Perma Ban (5,000 Days which is the equivalent of 13 Years).

Whatever it is you did that got you Perma Banned, you did it more than three weeks ago.

Again, based on Activision's Banning Policy, Bans are irreversible.

maybe your account was hacked, and the hacker got banned from your account.
