to get ruthless killer medal is it ONLY weapon kills?

it probably has been asked before, but just want to comfirm it since ive gotten over 20 on occassions but with killstreaks

    Not necessarily. To better answer your question, you can get the achievement with both gun & equipment kills. Scorestreak kills however do not count towards the kills.

     

    Tip*** Use only one Scorestreak - UAV's.
